由Smikestein设计，共5项，包含5个维度，分别是：适应度、协作程度、成长性（Growth）、亲和度、亲密度。采用Likert 3点计分，0代表“几乎很少”，2代表“经常这样”。所有条目之和就是一个总得分，分数越高，表明家庭功能越好。总得分7～10分、4～6分、0～3分，分别代表调查者家庭功能良好、中度障碍、严重障碍。
